Health History — Interoperability

 

Hometown Health is focused on member access to their health history a seamless process by removing barriers between patients, doctors, hospitals, and insurance companies. This process delivers the information using the current industry standards for data management (FHIR 7), has safeguards to ensure privacy, and is efficient.

Patient Access APT (July 1, 2021): Enables members to access up to five years of their health history in consumer applications, such as third party mobile applications.

Provider Directory API (July 1, 2021): Enables members to access their providers’ data when needed to ensure consistency of treatment and care coordination.

Payer to Payer API (January 1, 2022): Allows members to request their health history to be transferred from an old provider to a new provider.
